What is included in the monthly rate
One rate by assessed acuity. Incontinence supplies and local medical transportation are included, not billed separately.

- Three cooked meals plus snacks, diet-specific

- Medication administration and weekly log audit
- Laundry, housekeeping, and linen service

Pricing & requirements
From $4,250 / month
- Prerequisites
- A completed physician's report (Form DMA-6 or equivalent), current medication list, and a negative TB screening within 12 months.
- Timeline
- Assessment within 48 hours. Move-in typically 5–10 days after approval.
- Payment
- Private pay, long-term care insurance, and VA Aid & Attendance. We provide itemized carrier documentation monthly.

Evening medication pass, lower lighting, quieter household routine.
